29 CFR 1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:       Location: 320 N. 4th Street, Tipp City, OH, on April 14, 2014, employees were operating a Mitsubishi Forklift Truck Model # FGC15N had not received training required by this subpart.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(q)(7): Powered industrial truck(s) which were used on a round-the-clock basis were not examined for defects prior to or after each shift:    Location: 320 N. 4th Street, Tipp City, OH, employees did not perform daily inspections on the Mitsubishi Forklift Truck Model # FGC15N, to identify damage or other unsafe conditions to include those items listed in the operators manual.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
