29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On April 15, 2014 employees were not protected from a fall while roofing a residential property, exposing employees to a fall hazards of 13 feet.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person.   On April 15, 2014 an employee was exposed to a fall as the anchor was not installed under the supervision of a qualified person and not capable of supporting 5,000 pounds.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
