Randolph Dr, Philadelphia, PA The employees were working off of a fabricated frame scaffold approximately 120 feet long, 38 feet high and 5 feet wide which was not fully planked or decked on all working levels between the front uprights and the guardrail supports. This hazard exposed the employees to fall hazards ranging from approximately 13 to 39 feet. This condition was observed on April 11, 2014

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access: a) 901 N. Randolph Dr, Philadelphia, PA The employees were working off of a fabricated frame scaffold approximately 120 feet long, 38 feet high and 5 feet wide which did not have proper access for the employees to reach the various working levels of the scaffold. This hazard exposed the employees to fall hazards. This condition was observed on April 11, 2014
