Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure the process hazard analysis addressed the hazards of the process.

Identified process piping and vessels include but are not limited to:    a.     1359-10-B1-20 (Circuit From PV-1223 to E-3202/3) b.     16-HC-111-B1 (E-3185 De-ethanizer Reflux Chiller Pipe Circuit) c.     1359-141-A1-4 (E-3209 Pipe Circuit) d.     PV-1223 (De-propanizer Column) e.     Refinery Fuel Gas Line    B.    The violation occurred in the Fractionation Plant on or about April 17, 2014 and at times prior thereto where employees were exposed to fire and explosion hazards when the employer failed to ensure testing and inspection procedures of identified safeguards such as the Plant Emergency Shutdowns (ESD-350A-F) were conducted in accordance with RAGAGEP such as ISA-84.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure inspections and tests procedures are performed to maintain the on-going mechanical integrity of process equipment followed recognized and generally accepted good engineering practices (RAGAGEP).
