Prior to the move, this scaffold was not restrained from tipping as its height was approximately 542 inches (approximately 45.17 feet) and the widest section of its base was approximately 120 inches (approximately 10 feet). Condition existed on or about 04/15/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. This scaffold was not moved and erected under the supervision and direction of a competent person and, as such, contained the following deficiencies: 1) was not restrained from tipping as its height was approximately 542 inches (approximately 45.17 feet) and the widest section of its base was approximately 120 inches (approximately 10 feet) - greater than the 4:1 ratio, 2) did not contain a rest platform for the scaffold's attached ladder, 3) contained an un-rated rope attached to the scaffold's upper hand rail by a single knot and was used by employees for fall protection when accessing/egressing the scaffold's attached ladder, and 4) scaffold was moved with an employee on its upper walking/working surface. Condition existed on or about 04/15/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
