29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace.    a) 2355 Mohawk Lane- The employer failed to ensure that personal protective equipment used by employees, such as the fall protection harness and ropes, were free of damage such as frayed areas of the equipment.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment as outlined in (i)-(v) of this section:    a) 2355 Mohawk Lane- The employer failed to provide training on the use of, inspection of and care of personal protective equipment, such as fall protection equipment, that they are required to use by this section.    b) 2355 Mohawk Lane- The employer failed to provide training on the use of, inspection of and care of personal protective equipment, such as hard hats, that they are required to use by this section.    c) 2355 Mohawk Lane- The employer failed to provide training on the use of, inspection of and care of personal protective equipment, such as eye protection, that they are required to use by this section.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    a) 2355 Mohawk Lane- The employer failed to require the use of head protection, such as hard hats, for employees that were exposed to falling objects and debris during tree trimming operations.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
