29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:       a) Southwest Parking lot: Employee without the use of protective helmet was receiving a gas powered blower from the employee on top to the truck being exposed to an overhead struck by hazard, on or about 4/11/14.     b) On scaffold on Southwest Parking lot: Employee without the use of protective helmet was engaged in roofing work while standing on a scaffold platform with other employees on a higher platform, being exposed to an overhead struck by hazard, on or about 4/11/14.     c) On scaffold on Southwest Parking lot: Employee without the use of protective helmet coming down the scaffold while other employees were engaged in roofing activities on higher platforms, being exposed to an overhead struck by hazard, on or about 4/11/14.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a) Southwest Parking lot: Employee without any means of fall protection was standing on top of a moving box truck, being exposed to a fall of approximately 11 feet onto an asphalt driveway, on or about 4/11/14.
