(a)  On or about 4/18/2014, on the east side of the building, employees working from a job-made work platform elevated by a JLG 6042 telehandler were exposed to a fall hazard of more than 10 feet to the ground below. The platform in use was not approved by the manufacturer for capacity, construction and use.
