29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.    (a)  On or about 4/18/2014, on the east side of the building, employees working from a job-made work platform elevated by a JLG 6042 telehandler were exposed to a fall hazard of more than 10 feet to the ground below. The platform in use was not approved by the manufacturer for capacity, construction and use.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    (a)  On or about 4/18/2014, on the north end of the building, employees  working on a roof without fall protection were exposed to falling more than 15 feet to the ground below.
