29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 5 Forest Ridge Ct Second Level Phenix City, Alabama:  On or about April 17, 2014 and at times prior; the employer failed to ensure employees involved in residential roofing activities on the second level roof were using fall protection exposing them to a fall hazard of 20 feet to the ground below.  b)  At 5 Forest Ridge Ct First Level Phenix City, Alabama:  On or about April 17, 2014 and at times prior; the employer failed to ensure employees involved in residential roofing activities on the first level roof were using fall protection exposing them to a fall hazard of 9.8 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 5 Forest Ridge Ct Phenix City, Alabama:  On or about April 17, 2014 and at times prior the employer failed to ensure a portable extension ladder used by employees to gain access to and from the first level roof extended at least three feet past the landing area exposing employees to a fall hazard of 9.8 feet to the ground below.  b) At 5 Forest Ridge Ct Phenix City, Alabama:  On or about April 17, 2014 and at times prior the employer failed to ensure a portable extension ladder used by employees to gain access to and from the second level roof extended at least three feet past the landing area exposing employees to a fall hazard of 9.8 feet to the ground below.
