29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. (a) Front of House - On or about April 17, 2014 employees were exposed head injuries while an employee was working on a scaffold 17 feet 8 inches above them. Employee dropped a metal triangle to the ground. None of the employees were wearing hardhats.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway was not at least 18 inches (46 cm) wide. (a) Front of House - On or about April 17, 2014 an employee was exposed to a fall hazard while working on a wooden scaffold with walkway less than 18 inches. The scaffold walkway was 8 inches wide. Employees were working 17 feet 8 inches above concrete.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection was not provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. (a) Front of House - On or about April 17, 2014 an employee was exposed to a fall hazard while working on a wooden scaffold 17 feet 8 inches above concrete with no guardrail system or personal fall arrest system.
