29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Tire Liquidation LLC, Shreve, Ohio:  On or about December 23, 2013, at the address of 352 South St., Galion, Ohio, the employer failed to develop and implement a hazardous energy control program, which included procedures and training to protect employees from injuries while they engaged in servicing and maintenance activities.  Employees were exposed to crushing and amputation hazards while performing tasks such as, but not limited to clearing rubber jams from the SSI manufactured tire shredder (#1).
