29 CFR 1910.146(c)(5)(ii)(C): Employees entered into permit spaces that met the conditions set forth in 29 CFR 1910.146(c)(5)(i) but before an employee entered the space, the internal atmosphere was not tested with a calibrated direct-reading instrument. a) On or about April 2, 2014, at the P2 pulpit scarfing pit, located in the Rolling Mill at The Timken Company Faircrest Ohio plant, the employer allowed an employee to enter into a pre-designated permit required confined space, to retrieve a lock that he dropped, without first having the atmosphere tested for oxygen deficiency or flammable gases and vapors. b) On or about April 2, 2014, at the P2 pulpit scarfing pit, located in the Rolling Mill at The Timken Company Faircrest Ohio plant, the employer instructed an employee to enter into a pre-designated permit required confined space to hook the overhead crane up to the scarfing pit lid sling, to lift the lid out of the pit, after it had fallen in. The atmosphere was not tested for oxygen deficiency or flammable gases and vapors prior to the employee entering the space.
