29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.    a) On and before April 2, 2014, at the P2 pulpit scarfing pit, located in the Rolling Mill at the Timken Faircrest Ohio plant, the employer did not have a visible danger or confined space sign posted near the scarfing pit a designated permit required confined space in which employees was instructed to enter by a supervisor.

29 CFR 1910.146(c)(5)(ii)(C): Employees entered into permit spaces that met the conditions set forth in 29 CFR 1910.146(c)(5)(i) but before an employee entered the space, the internal atmosphere was not tested with a calibrated direct-reading instrument.     a) On or about April 2, 2014, at the P2 pulpit scarfing pit, located in the Rolling Mill at The Timken Company Faircrest Ohio plant, the employer allowed an employee to enter into a pre-designated permit required confined space, to retrieve a lock that he dropped, without first having the atmosphere tested for oxygen deficiency or flammable gases and vapors.      b) On or about April 2, 2014, at the P2 pulpit scarfing pit, located in the Rolling Mill at The Timken Company Faircrest Ohio plant, the employer instructed an employee to enter into a pre-designated permit required confined space to hook the overhead crane up to the scarfing pit lid sling, to lift the lid out of the pit, after it had fallen in. The atmosphere was not tested for oxygen deficiency or flammable gases and vapors prior to the employee entering the space.

NOTE: ANY TIME THAT AN ENEGRY ISOLATING DEVICE IS USED ALL RESIDUAL ENERGY MUST BE VERIFED TO HAVE BEEN RELEAVED OR DISAPATED.

On and before April 7, 2014, at The Timken Company Faircrest Ohio plant, the employer did not certify in writing, to include; the machine or equipment, date, employees included, and person performing the inspection, for  periodic inspections of maintenance employees who perform hazardous energy control in the Rolling Mill.
