29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.     A. Ground Solutions LLC worksite located at Harbor Town Place, Perrysburg, Ohio: On or about April 17, 2014, the employer did not ensure employees were protected from head injuries while working near operating excavators while performing trenching activities. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or struck-by hazards.

Ground Solutions LLC worksite located at Harbor Town Place, Perrysburg, Ohio: On or about April 17, 2014, the employer did not ensure a safe means of egress such as a ladder was provided to employees installing a storm sewer structure and pipe in an excavation approximately 8.00 feet deep.
