29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: On or about April 21, 2014 and times prior thereto, at the workplace located at or near 351 Lake Hamilton Drive, Hot Springs, Arkansas, the emergency exit door was locked with a metal bar and screws. This hazard exposed employees to the hazard of smoke inhalation in the event a fire occurred.

On or about April 21, 2014 and times prior thereto, at the workplace located at or near 351 Lake Hamilton Drive, Hot Springs, Arkansas, the breaker box located in the downstairs janitor's closet had an opening in the top right side, exposing employees to electrical hazards.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  On or about April 21, 2014 and times prior thereto, at the workplace located at or near 351 Lake Hamilton Drive, Hot Springs, Arkansas, the storage are underneath of the movie screens as well as the storage area located on the second level in the vicinity of the projection room contained food waste such as candy and popcorn as well as rodent feces. This condition exposed employees to the health hazard of rodent-borne disease.
