29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) while working from an aerial lift.   On 22 April 2014, and at times prior thereto, an aerial lift was being used to lower or raise workers within the basket without those workers having a safety harness with a lanyard and being tied off to the boom or basket, exposing the workers to being ejected from the basket and falling approximately 14 feet to the ground or concrete below.

29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded.    On 22 April 2014, and at times prior thereto, an aerial lift was being used to lower or raise three workers within the basket, exceeding the manufacturer's load limits and exposing the workers to the lift toppling over and falling approximately 14 feet to the ground or concrete below.
