29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):  a)  On or about 4-13-14, the employees engaged in installing a new manhole and making sanitary sewer connections at a depth greater than five (5) feet were not provided any protection against accidental side wall collapse of the excavation in that no shielding system or benching or sloping method were used.
