29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage:  On or about at 04/23/14 located at 701 Park Haven LN., in Canton, GA: employees were operating a skill saw, in which the power cord insulation was damaged while cutting 2 x 4s and 2 x 6s lumber.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about at 04/23/14 located at 701 Park Haven LN., in Canton, GA: the employer failed to provide personal fall protection for employees that were exposed to fall hazards of up to 30 while working on the 2nd floor installing ceiling joist.
