29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    On or about April 23, 2014 and times prior thereto, at the worksite located at or near 14 S Shore Circle, Maumelle, Arkansas, two of the extension cords used to provide electricity to the power tools used in the construction of an enclosed deck had the insulation pulled away from the cord plug. This condition exposed employees to electrical hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about April 23, 2014 and times prior thereto, at the worksite located at or near 14 S Shore Circle, Maumelle, Arkansas, employees were working on installing roof framing for an enclosed gazebo at a height greater than 6 feet from their location to the ground below. This condition exposed employees to the hazard of falls from an elevation.
