29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:    a) Third floor, on or about 4-21-14:  Employees were working in the area immediately adjacent to a stair opening, with no mid rail, exposing the employee to a fall hazard of 9 feet 5 inches.

29 CFR 1926.1052(c)(3)(ii): The height of stairrails installed before March 15, 1991, was less than 30 inches (76 cm) from the upper surface of the stairrail system to the surface of the tread, in line with the face of the riser at the forward edge of the tread:    a) First to second floor, on or about 4-21-14:  Employees were using a stairway with a wrought iron hand rail that was disconnected and at a height of 26-1/2 inches, exposing the employees to a fall hazard of 6 feet 7 inches.

29 CFR 1926.1052(c)(5): Handrails and the top rails of stairrail systems were not capable of withstanding, with failure, a force of at least 200 pounds (890 n) applied within 2 inches (5 cm) of the top edge, in any downward or outward direction, at any point along the top edge:    a) First to second floor, on or about 4-21-14:  Employees were using a stairway with a wrought iron hand rail that was disconnected and not able to withstand a 200 pound load applied in any direction, exposing the employees to a fall hazard of 6 feet 7 inches.
