29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed:  In the control room, where employees were scooping ice from a Manitowoc ice machine, an empty plastic coffee container with inset grip was provided in lieu of a scoop that could be returned to the holder for sanitary reasons, exposing employees to the hazards of bacterial contamination.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  In the control room, where employees were scooping ice from a Manitowoc ice machine, the flexible cord powering the machine had the outer sheath pulled back from the plug and insulation was frayed with conductors subject to to physical damage, exposing employees to the hazards of electrical shock.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:  (a) Outside, in the Cullett Area, where employees were filling the fuel tank on the front end loader with combustible diesel fuel, the tank was not labeled with hazard warnings.   (b) Outside, in the Cullett Area, where employees were loading recycled glass into a hopper for crushing and mixing with raw ingredient(s), a raw ingredient tank was not labeled, exposing employees to the hazards of chemicals.
