29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    Wet Room:    A job-made hand lamp was not approved by a nationally recognized testing laboratory (NRTL) to be used for illuminating the interior of 55-gallon drums that may contain flammable vapors.  On or about April 16, 2014, an employee sustained a thermal burn from a flash fire created by the flammable vapors within a metal drum that previously contained cyclohexane which ignited when the job-made lamp was used to illuminate the drum's interior.
