29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    WAREHOUSE:  The employer had not verified through a written certification that the workplace hazard assessment for personal protective equipment (PPE) had been performed as complete, i.e., fall protection for the powered industrial truck operators.    Wusthof Trident of America was previously cited for a similar violation of 29 CFR 1910.132(d)(1), which was contained in Citation 1, Item 1, contained in OSHA inspection number 123191330 issued on 11-30-2009 with a final order date of  12-22-09 with respect to a workplace location at 355 Wilson Avenue, Norwalk, CT 06854.
