29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a). Worksite located at 1402 Cinnamon Hill Lane, Columbia, Missouri- Employees were working on a scaffold in which the employees were having to duck in order not to hit their head on the awning. Employees were also exposed to overhead hazards when ascending or descending from the scaffold and were not wearing hard hats.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  (a). Worksite located at 1402 Cinnamon Hill Lane, Columbia, Missouri- Employees were exposed to a fall hazard in that the platforms the employees were working from were not fully decked or planked.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (a). Worksite located at 1402 Cinnamon Hill Lane, Columbia, Missouri- Employees were exposed to fall or scaffold collapse hazard in that the scaffold base plates were not bearing on mudsills to prevent the displacement and settling of the legs of the scaffolding. Many of the base plates were buried in the mud.

Worksite located at 1402 Cinnamon Hill Lane, Columbia, Missouri- Employees were exposed to fall hazards in that the scaffold was not provided with any means of fall protection such as  standard guardrailing.
