29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the workplace hazard assessment required in 29 CFR 1910.132(d)(1).    Location: Small parts casting department and main paint booth.    Workers in the small parts casting department were working with 2-part urethane elastomers containing 4,4-Diphenylmethane diisocyanate (MDI), a chemical that can cause both skin and respiratory sensitization, and they were not wearing sleeves to cover their exposed arms.  Also, 1 employee was wearing shorts.    An employee spray painting paint with organic chemicals was not wearing gloves.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    Location:  Worksite    Employees wore half-face negative pressure respirators on a voluntary basis and had not been provided with medical evaluations to ensure that they could wear them safely.
