29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: On or about 04/25/2014, while completing the demolition of a pool enclosure/structure from the top of a flat roof on a commercial building, located at 710 White Pond Dr. in Akron, OH, employees were not protected from a fall hazard of (approximately) 20 - 25 feet, by a fall protection system and/or device.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards: On and before 04/25/2014, at the address of 710 White Pond Dr., Akron OH, the employer failed to ensure that employees completing the demolition of a pool enclosure/structure from the top of a flat roof on a commercial building were trained in the appropriate use of fall protection. Equipment includes, but is not limited to, personal fall arrest systems or devices.
