29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:     Worksite: Employees performing roofing activities in excess of 10 feet in height were not adequately protected from falls.  One employee wore a fall protection harness but did not connect it to the protective device, while another employee wore no harness at all.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective was not removed from service until repaired:  Worksite: The employer did not visually inspect, at least daily, a portable cross-cut power saw which had splits, cuts, and dry-rot in the insulation of the attached flexible power cord. The exposed internal conductors presented shock and electrocution hazards to employees.
