29 CFR 1926.1052(c)(1)(ii): One stair rail system along each unprotected side or edge: a) On or about April 23, 2014 - at a jobsite located at 862 N. Franklin Avenue, Chicago, IL - employees performing construction activities were using a staircase that did not have a stair rail on each unprotected side(s), thereby exposing the employees to fall hazards of approximately ten (10) to fifteen (15) feet to the lower level.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings shall be provided with guardrail systems. Guardrail system criteria are contained in subpart M of this part: a) On or about April 23, 2014 - at a jobsite located at 862 N. Franklin Avenue, Chicago, IL - employees performing construction activities was using a staircase landing that was not equipped with guardrail(s) along all unprotected side(s), thereby exposing the employees to fall hazards of approximately ten (10) to fifteen (15) feet to the lower level(s).

29 CFR 1926.502(b)(9): Top rails and midrails shall be at least one-quarter inch (0.6 cm) nominal diameter or thickness to prevent cuts and lacerations. If wire rope is used for top rails, it shall be flagged at not more than 6-foot intervals with high-visibility material. a) On or about April 23, 2014 - at a jobsite located at 862 N. Franklin Avenue, Chicago, IL - the wire rope guardrail system on the perimeter of the building did not have flags for proper visibility, thereby exposing employees to fall hazards.
