29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:   a) On or about 4/24/14, on the Sales Room Floor in the Save-A-Lot store under renovation at 4271 Lake Ave., Blasdell, NY: A metal 4-receptacle outlet box attached to the end of a flexible cord was laying on the floor. This is prohibited by Articles 370-13, and 370-5 of the National Electric Code. Four electrical cords were plugged into the receptacles on this box including an extension cord that supplied power to the JCSmith portable industrial fan in the back room.   b) On or about 4/24/14, in the back room of the Save-A-Lot store under renovation at 4271 Lake Ave., Blasdell, NY: A metal 2- receptacle outlet box was suspended in the air, supported solely by a flexible cable. This is prohibited by Article 370-13 of the National Electric Code. A large diameter red extension cord owned and brought on the jobsite by K.M. Smith General Contracting, Inc. was plugged into a receptacle on this box, and it supplied power to a 2-Light Portable Work Light Stand in the Mechanical Room that belonged to the Refrigeration Contractor.   NO ABATEMENT CERTIFICATION REQUIRED

The plug on the power cord of the fan lacked a ground prong.  b) On or about 4/24/14, in the mechanical room of the Save-A-Lot store under renovation at 4271 Lake Ave., Blasdell, NY: A 2-Light Portable Work Light Stand belonging to the Refrigeration Contractor was connected to 120-volt ac power via a red extension cord that was plugged into a receptacle in a 2-receptacle outlet box. The plug on the red extension cord lacked a grounding prong, and was owned and brought on the jobsite by K.M. Smith General Contracting, Inc.    NO ABATEMENT CERTIFICATION REQUIRED
