29 CFR 1926.100(a): The employer did not ensure that employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were protected by protective helmets.  This violation occurred on March 27, 2014, at a job site located at 108 W. Ervin, Tyler, Texas, when employees working with a crane lifting HVAC equipment on to a roof were not protected by the use of hard hats.

29 CFR 1926.1053(b)(1): The employer did not ensure that when portable ladders were used for access to an upper landing surface, the ladder side rails extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  This violation occurred on March 27, 2014, at a job site located at 108 W. Ervin, Tyler, Texas, when employees used an extension ladder to gain access to a roof.  Employees were exposed to a fall of approximately 20 feet to the ground below.
