29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  This violation occurred on or about April 24, 2014, at a jobsite located at 4490 Eldorado Parkway, McKinney, Texas; where the employer did not train employees lowering appliances from the balcony of a second story unit in the recognition and avoidance of unsafe conditions. Employees setting appliances on a wooden box set on the elevated forks of a rough-terrain forklift were exposed to struck-by and fall hazards.
