29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  (a) 18 Industrial Way, Denver Pennsylvania - An employee used an elevated platform to cross over equipment.  The employer did not ensure that swinging gates were provided along the side of the platform, on or about April 7, 2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The form did not maintain the employees job title and department, the classification of the case, the number of days the employee was away from work and/or transferred or restricted and the type of injury, on or about April 28, 2014. 29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) 18 Industrial Way, Denver Pennsylvania - Employees that work with hazardous chemicals including paint thinners and oils were not provided with the required training on the changes in the hazard communication program such as labeling, pictograms and safety data sheets, on or about April 28, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
