29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On April 24, 2014, at this job site, the employer failed to provide fall protection to guard against open side ends and falling forward as employees worked on a supported scaffold while applying stucco onto the exterior of the building , at heights exceeding 18 feet above the ground level.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At this job site, at the base and entrance of the external wall of the scaffolding, employees were exposed to tripping hazards where broken masonry, trash and other materials and debris were not kept out of walkways as the stucco work progressed.
