29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure:  a.  Within the facility, on or about 4/29/14:  Two employees having potential exposure to blood and/or other potentially infectious material (OPIM) were not provided the hepatitis b vaccination series within ten days of initial assignment.  Nor had the employees signed declination form.  Abatement certification must be submitted for this item.  The Northwoods at Moravia dba Howd Nursing Home was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1030(f)(2)(I), which was contained in OSHA inspection number 312373590, citation number 1, item number 4 and was affirmed as a final order on 04/01/11, with respect to a workplace located at 7 Keeler Avenue, Moravia, NY 13118.
