29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location:  177 Commercial Street, Braintree, MA  Employees were exposed to fall hazards of over 20 feet from the rooftop to the ground below, where a competent person did not adequately inspect the jobsite to ensure that employees were wearing personal protective equipment, or that the jobsite was free from hazardous conditions.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees were exposed to fall hazards of over 20 feet to the ground below where fall protection had not been used or provided in the following instances:  Instance 1) Sheathing an 8/12 pitched roof without fall protection  Instance 2) framing and cutting where window openings had not been guarded
