29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  (a)  Throughout Facility - On or about April 24, 2014, employees who perform servicing and maintenance on equipment and machinery with hazardous energy sources, such as but not limited to 40-Ton Injection Presses, Vertical Injection Presses, Horizontal Injection Presses, Knockout Presses, and Trimmers, were not provided with training to the authorized level, in that they were not trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)  Clean Room - On or about April 24, 2014, employees operating the 40-Ton Injection/Compression Presses were exposed to hazards created by the point of operation, in that the point in which the Injection Piston pushes material into the Injection Pot in the Top Heating Platen was not guarded.
