29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: On or about April 30, 2014, employees were exposed to a fall of greater than six feet to the ground below, while they worked framing and laying decking on the second story floor of a residential construction, without any form of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling, and did not train each employee in the procedures to be followed in order to minimize these hazards: On or about April 30, 2014, employees were exposed to a fall of greater than six feet to the ground below, while they worked framing and laying decking on the second story floor of a residential construction, without being trained in the hazards of this practice.
