(b) Sewing Room - On or about April 25, 2014, a live receptacle located on a column approximately 24 inches from a work table was damaged in that the face of the receptacle was cracked and broken, exposing internal connections.

29 CFR 1910.334(a)(3)(iii): Adapters which interrupt the continuity of the equipment grounding connection were used:  (a)  Sewing Room, Station 3 - On or about April 25, 2014, an employee was exposed to potential electric shock hazards while operating a sewing machine, powered by an extension cord connected in series with a Relocatable Power Tap, which was plugged into a non-grounded duplex two-blade receptacle with the use of three-prong adapter plug.  (b)  Sewing Room, Station 2 - On or about April 25, 2014, an employee was exposed to potential electric shock hazards while operating a sewing machine powered by a Relocatable Power Tap connected in series with a two-blade extension cord with a three-prong adapter plug, which was plugged into a non-grounded duplex two-blade receptacle.
