(b) Sewing Room, Station 2 - On or about April 25, 2014, an employee was exposed to potential electric shock hazards while operating a sewing machine powered by a Relocatable Power Tap connected in series with a two-blade extension cord with a three-prong adapter plug, which was plugged into a non-grounded duplex two-blade receptacle.
