29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations resent potential eye or face injury from physical, chemical or radiation agents: a) On or about April 30, 2013, at a jobsite located at 466 South 162nd Street, South Holland, IL. - employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to hazards associated with eye injuries.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) On or about April 30, 2013, at a jobsite located at 466 South 162nd Street, South Holland, IL. - employees performing roofing activities on a roof with a 4:12 pitch were not protected from falls, thereby exposing the employees to fall hazards of approximately seven (7) feet four (4) inches.
