29 CFR 1910.1025(h)(1):  All surfaces were not maintained as free as practicable of accumulations of lead:      1) On April 30, 2014, a conveyor cover on the main line was covered with a layer of dust containing 0.33% lead.      2) On May 9, 2014, a surface directly next to the main line picker employee had a layer of dust containing 0.29% lead.      3) On May 9, 2014, a surface next to the main walkway across from the bathroom had a layer of dust containing 0.50% lead.      Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.146(c)(2):  The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:   1) On May 9, 2014, and at prior times, the employer had not posted signs warning of the danger of entering the spaces on the hatch entryways of the dust collection bins.    Abatement note: A sign reading "DANGER- PERMIT REQUIRED CONFINED SPACE, DO NOT ENTER" would satisfy the requirement of this section.   Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
