29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  a. The lockout program annual inspection was not documented/certified to show that the annual inspection/audit had been performed.  (Note: The employer's lockout program identified the requirement for an annual certification with a form included in an appendix of the program.  The annual inspection requires review of written procedures, such as energy sources, where to lockout, how to test, alternative means of protection, inspection of lockout equipment, and ensuring that employees are performing lockout as required.)  Abatement documentation is required for this violation.
