Condition observed on or about 5/1/14.   b) The employer failed to provide abatement and certification for abatement of citation 1 item 4 in which ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could effect there safe use. Condition observed on or about 05/1/14.  c) The employer failed to provide abatement and certification for abatement of citation 1 item 6 in which employees had not received ladder safety training. Condition observed on or about 05/1/14.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  a- 47 Binsted Drive, Medford NJ: On or about 05/1/14, company employees performing roof sheathing installation and other related activities, from a roof with an approximate pitch of 7 on 12, were exposed to falls in excess of 10 feet 3 inches to the ground below when they were either not wearing any fall protection or not properly utilzing the fall protection they were supplied with.  In addition, these company employees had not received any fall protection training.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a- 47 Binsted Drive, Medford NJ:  Company employees were exposed to fall hazards in excess of 10 feet 3 inches to the ground surface below when utilizing aluminum extension ladders that; contained multiple rungs that were bent/damaged, were missing the ropes used to extend the fly sections, containing a visibly broken locking mechanism for the fly section, and were set into unstable materials causing them to sink into the ground below.  Conditions observed on or about 05/1/14.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

Multiple rungs on the ladders were bent/damaged, both ladders were missing the ropes used to extend the fly sections, and one ladder was being used with a visibly broken locking mechanism for the fly section.  Condition observed on or about 05/1/14.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards.  a- 47 Binsted Drive, Medford NJ:  Company employees were exposed to fall hazards in excess of 10 feet 3 inches above the ground surface below when utilizing aluminum extension ladders that contained multiple bent/damaged rungs, were missing the ropes used to extend the fly sections, and were being used with a visibly broken locking mechanism for the fly section.  The exposed company employees had not received any training from the employer regarding ladder safety hazard recognition.  Condition observed on or about 05/1/14.    IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
