29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) Worksite, 14 Prospect Ave, Garden City NY - An employee was trimming tree branches while working from a 60ft aerial lift, model # AL60150, serial # 2594995. The employee was approximately 30ft above the ground and was not wearing a body belt or harness attached to the boom or basket; on or about 5/2/14. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

The employee was not wearing a protective helmet and was exposed to being struck by overhead tree branches; on or about 5/2/14. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
