The employee was approximately 30ft above the ground and was not wearing a body belt or harness attached to the boom or basket; on or about 5/2/14.     Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
