29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:     A. Extraordinary Exteriors worksite located at 7820 North Woodbridge Rd., Monclova, Ohio: On or about April 30, 2014, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns while installing weatherproofing on the west side of the two story residential structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    A. Extraordinary Exteriors worksite located at 7820 North Woodbridge Rd., Monclova, Ohio: On or about April 30, 2014, the employer did not ensure an employee was protected from head injuries while working below employees installing weatherproofing on the two story residential structure. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

Extraordinary Exteriors worksite located at 7820 North Woodbridge Rd., Monclova, Ohio: On or about April 30, 2014, the employer did not ensure employees were protected from fall hazards while installing weatherproofing on the west side of the two story residential structure. Employees were exposed to a fall distance of approximately 18.00 feet from the 9/12 pitched roof.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:      A. Extraordinary Exteriors worksite located at 7820 North Woodbridge Rd., Monclova, Ohio: On or about April 30, 2014, the employer did not ensure the side rails of the extension ladders being used on the west side of the two story residential structure were extended at least three feet above the landing surface. Three employees were exposed to falls while using the ladders to access the roof.
