29 CFR 1910.305(g)(1)(iv)(A):  Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: As a substitute for the fixed wiring of a structure;  a.  At the workplace located at 110 Straits Lane, Killbuck, Ohio, in the old sub arc area, bay area 3, a flexible cord was used as a substitute for fixed wiring to supply power to a Hitachi G12SE2 Disk Grinder, thereby exposing the employees to electrical shock and fire hazards.
