29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    On or about April 30, 2014, employees engaged in framing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 18 feet to the ground below.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:    On or about April 30, 2014, the stairs leading to the second floor were not equipped with handrails, exposing employees to the hazard of falling.
