29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  On or about May 1, 2014, at 1235 Sams Avenue, Harahan, LA, the employer failed to ensure that electrical cords were free from recognized hazards. Employee(s) were using the extension cord for as a power source for a portable drill.  Instance 1) The extension cord had over current damage. Instance 2) The extension cords outer casing was cut and the inner wires were exposed.

29 CFR 1926.453(b)(2)(v): A body belt (full body harness) was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about May 1, 2014, at 1235 Sams Avenue, Harahan, LA, the employer failed to ensure that employee(s) wore fall protection when working in the basket of the extendable boom aerial lift. The employee was observed working at a height of approximately 18 feet in a high traffic area.
