Three electrically powered vertical mixing tanks with vertical agitators, and 3000 gal. capacity,were not adequately guarded to prevent employees from falling into the mixers during dispensing, monitoring and washing operations. The covers provided did not have grates under them with openings only large enough to allow for hoses/brushes for washing the insides and for the raw material being dispensed. Observed on/or about 05/07/2014.

Approved Class II, Division 2 electrical receptacles were not used in that standard electrical outlet(s) and light switch(es) were provided Class II Division 2 electrical receptacle(s) and portable electric equipment were not used when located within 25 ft. of a Class II Division 2 location.
