29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:      A. Devos Excavating LLC worksite located at 501 W. Boundary St., Perrysburg, Ohio: On or about May 6, 2014,  the employer did not ensure an employee was adequately protected from cave-ins while installing a 12 inch waterline in a trench composed of "B" soil that varied in depth from 6.00 feet to 7.00 feet deep.
