29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  (a)  On or about 4/24/2014, on the south side of the duplex, debris and scrap lumber with protruding nails were not kept clear from the work areas, exposing employees to the hazards of lacerations and/or contusions caused by stepping or falling onto the debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about 4/24/2014, employees installing a new roof were exposed to unprotected falls of more than 6 feet to the ground below.
