29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: (a) Location - 29 Old Right Road, Ipswich MA: On or about 5/8/2014, valve technicians are required to access the top of a valve test stand that is greater than four feet above the lower level during valve testing and there is no guard rail.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Location - 29 Old Right Road, Ipswich, MA: On or about 5/8/2014, the safety shields installed on valve testing equipment were inadequate in that they did not form a full enclosure around the assembly under pressure exposing employees to the hazard of being struck by projectiles and/or test medium in the event of equipment failure.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) Location - 29 Old Right Road, Ipswich, MA: On or about 5/8/2014, the employer's assessment of the potential hazards created by valve testing did not account for the possibility of flying fragments from test assembly rupture and did not quantify the magnitude of identified potential test equipment failures to ensure the use of the personal protective equipment necessary to protect employees in the event of these equipment failures.
